Employment Forms

Paperwork submission:  Paperwork will be submitted on Sunday, April 28th from 12-2 pm.

REQUIRED PAYROLL, FEDERAL, AND STATE FORMS

  • Note that we are moving toward electronic submission of all payroll related forms beginning in 2024.
  1. Employees will need to return their employment agreement via email to [email protected] to indicate their acceptance of the position.
  2. Soon after this has been done, employees will receive an email from Paychex Flex inviting them to register and create an account on the Paychex Flex system.
  3. Once they have created an account, they will be prompted to complete their personal information, direct deposit information, and federal and state tax withholding forms.  Employees will no longer have to submit the paper versions of these forms.
  • All new employees must complete Form I-9, “Employment Eligibility Verification”, to enable the Club to verify the employee’s identity and employment authorization in accordance with Federal law. The I-9 form must be completed within 3 days of the employee’s first day of work.  Please perform the following:
  1. Review the I-9 instructions at https://www.uscis.gov/sites/default/files/document/forms/i-9instr.pdf.
  2. Print the I-9 form at https://www.uscis.gov/sites/default/files/document/forms/i-9.pdf.
  3. Complete Section 1 (and the Preparer and/or Translator Certification section on page 3 if applicable). Do not fill out Section 2, as this is the employer portion of the form.
  4. Bring the completed I-9 form and the required documentation to the employee paperwork collection session.  Note that the required documentation cannot be photocopies – only originals can be accepted per regulations.
  • All employees under the age of 16 must obtain a Youth Employment Certificate before they can begin working, in accordance with Virginia Department of Labor requirements. This process is now all online. It is a 5 step process as follows:
  1. Click here –https://vaeecs.doli.virginia.gov/vaeecs/– and complete the Youth Registration. You will receive an email with your Youth ID.
  2. Email your Youth ID to [email protected] along with a copy of your birth certificate and your lifeguard certification (if applicable).
  3. We will email you when we have completed the employer portion.
  4. Have a parent complete the Parent Guardian Registration.
  5. Sign (employee) the Youth Employment Certificate in order for it to be considered valid.

REQUIRED CHESTERBROOK FORMS

(Note that lifeguarding certification is required for any coach who is under the age of 16 per Virginia Department of Labor regulations.)

PLEASE NOTE: NO EMPLOYEE OR COACH CAN BEGIN WORKING UNTIL ALL FORMS ARE COMPLETED AND SUBMITTED.
