Employment Forms
Paperwork collection: I-9 Forms for new employees will be collected on Sunday, April 19th from 12-2 pm. All other forms are submitted online. Please do not fill out any forms until after receiving an offer of employment.
REQUIRED PAYROLL, FEDERAL, AND STATE FORMS
- Note that we use electronic submission of all payroll related forms except for the I-9 form required for new employees.
- Employees will need to return their employment agreement via email to [email protected] to indicate their acceptance of the position.
- Soon after this has been done, employees will receive an email from Paychex Flex inviting them to register and create an account on the Paychex Flex system.
- Once they have created an account, they will be prompted to complete their personal information, direct deposit information, and federal and state tax withholding forms within Paychex Flex.
- All new employees must complete Form I-9, “Employment Eligibility Verification”, to enable the Club to verify the employee’s identity and employment authorization in accordance with Federal law. The I-9 form must be completed within 3 days of the employee’s first day of work. Please perform the following:
- Review the I-9 instructions at https://www.uscis.gov/sites/default/files/document/forms/i-9instr.pdf.
- Print the I-9 form at https://www.uscis.gov/sites/default/files/document/forms/i-9.pdf.
- Complete Section 1 (and the Preparer and/or Translator Certification section on page 3 if applicable). Do not fill out Section 2, as this is the employer portion of the form.
- Bring the completed I-9 form and the required documentation to the employee paperwork collection session. Note that the required documentation cannot be photocopies – only originals can be accepted per regulations.
- All employees under the age of 16 must obtain a Youth Employment Certificate before they can begin working, in accordance with Virginia Department of Labor requirements. This process is completed online. It is a 5 step process as follows:
- After you receive an offer of employment, click here –https://selfserviceportal.doli.virginia.gov/LaborLaw/ChildLaborPermits/– and complete the Youth Registration. You will receive an email with your Youth ID.
- Email your Youth ID to [email protected] along with a copy of your birth certificate and your lifeguard certification (if applicable).
- We will email you when we have completed the employer portion.
- Have a parent complete the Parent Guardian Registration.
- Sign (employee) the Youth Employment Certificate in order for it to be considered valid and return the signed certificate to [email protected].

(Note that lifeguarding certification is required for any coach who is under the age of 16 per Virginia Department of Labor regulations.)
